Please print this application out or pick up a copy at either Activities Desk or the Clubhouse Media Center and complete prior to coming to the Clubhouse or Health and Fitness Center (HFC). The RFID tag will be installed on the driver's side headlight and must be done at the time of distribution, before leaving. Lake Wales residents will be scheduled to come to the Clubhouse by street starting September 10.
