CARE MANAGEMENT SPECIAL HEALTH CARE NEEDS Members with chronic physical, developmental, behavioral or emotional conditions are identified as Member with Special Healthcare Needs (MSHCN). Members designated with complex health needs can speak with our team about integrated care and treatment Service Management. Members have direct access to a Community Health Choice Care Management team for assistance with care coordination.
