CARE MANAGEMENT 3 MAKE FITNESS FUN FOR THE WHOLE FAMILY According to the Centers for Disease Control (CDC), children and adolescents should have 60 minutes or more of physical activity daily. Adults should do at least 150 minutes a week of moderate-intensity, or 75 minutes a week of vigorous-intensity aerobic physical activity. Aerobic types of exercise: Walking/Jogging - Take the entire family, including the pets, for a walk/jog at your local park. Cycling - Plan a 5-mile bike ride with the entire family. Muscle-strengthening types of exercise: Rope Climbing, Rock Climbing - Plan a rope/rock climbing outing with the family. Sit-Ups - Compete with a family member to see who can do the most sit-ups. Bone-strengthening types of exercise: Hopping, Jumping, Skipping - Compete with a family member to see who can do the most jumping jacks. Read the instructions carefully. Medicine may be prescribed to be given in either milliliters (ml) or teaspoons. One teaspoon is almost five times more than one milliliter. That is a big difference! In addition, medicine should come with a dosage tool (cup, syringe, etc.) so that you give the correct amount. If it does not, ask for one! If the medicine is to be given by teaspoon, do not use a kitchen spoon. It may not be an accurate measurement. WELLNESS 7 COMMUNITY REWARDS FOR MOM & BABY FOR STAR MEMBERS Community has launched a new Community Rewards program for Mom & Baby! In this program, members get rewarded for healthy activities! For mom: • A $25 gift card for early prenatal visit (within 6 weeks of becoming a member) • A $25 gift card for timely postpartum visit (3 to 8 weeks after delivery) Who is this for? This program is for Moms and Babies in our STAR program. New moms with pregnancy Medicaid and newborn STAR babies are eligible. How do I get started? You will get a phone call to help you sign up. So make sure we have your correct phone number so we can call you. You can also activate online through your My Member Account. How do I get rewarded? You get rewarded when you tell us that you have completed your activities in the correct time period. The easiest way to tell us is through your My Member Account. WELLNESS 8 HEALTHY LIVING IN THE NEW YEAR Texas Health Steps Checkups and Well-Child checkups, also known as wellness checkups, are the best way to help your child stay healthy as they grow. Medicaid Members get Texas Health Steps Checkups from birth through 20 years of age. CHIP Members get Wellness Checkups from birth through 18 years of age. These checkups are at no cost to Medicaid and CHIP Members. When are the checkups due? Within five days after being born Within 60 days after each checkup age listed below: 2 weeks 9 months 2 years 2 months 12 months 2 ½ years 4 months 15 months 6 months 18 months Once a year between 3 – 20 years of age, on or after each birthday Call your child’s doctor today to schedule an appointment. WELLNESS 9 CHILDREN OF TRAVELING FARMWORKERS If you have travelled to work on a farm or fields to prepare crops, grow fruits and vegetables, plant trees, raise or care for livestock, or prepare dairy products in the past 2 years then your children ages birth up to the day of their 18th birthday are considered Children of Traveling Farmworkers. Your children can receive their wellness checkup before they travel with you to the next farm job.
